- PHYSICIAN EXAMINATION FORM
To be used for all new students and those current students who need an updated physical during the summer months. All exams on file must not be older than 12 months at any given point during the school year. If the student does not have a current physical on file, he/she will not be eligible to participate in athletics or any other school activity. If you are unsure when your child will need an updated physical exam, contact the school nurse at 301-983-5744. - IMMUNIZATION RECORD
This document is required for all new students. - STUDENT INFORMATION SHEET
This is required only at the beginning of the year and must be on file before the start of school or before pre-season if your child is playing fall sports. - MEDICATION AUTHORIZATION CHANGE FORM
This is required to be on file with the nurse’s office when a student changes medications or dose amount during the school year whether taken at home or at school.
